I have 5 months child who's head always bit hot, and hands and foots relatively cold. Do I need to worry ? Please suggest me. Thanks in advance.

The heads of all babies tend to be warmer than the rest of the body. This is normal because the head receives more blood. In order to check if a baby has fever, you'll need to use a thermometer.
