Hair loss and hair thinning

Up until my age was 13-14 my hairs were really thick and dense. Then the hairfall started after which it has increased now upto that extent that i can see the scalp at the front, back and the left side of the skull. The hairs are really patchy and are considerably thin. I am 19 now. I am really worried about my condition. Please help.
