Your MRI shows a peri-anal abscess (pus collection) but no formed fistula tract yet. This is actually a good stage, because a fistula has not developed.
Even when MRI says “no fistula,” surgery may still be required because pus does not resolve fully with medicines alone. If it is not drained, it can persist or later turn into a fistula.
That is why your doctor has advised incision and drainage, not fistula surgery.
No muscle involvement mentioned is a positive sign, and early drainage usually heals well with sitz baths and local care.
Delaying treatment can increase future fistula risk.
