Yes it has to be operated. Hemorrhagic cyst has to be removed, as the cyst size is around 4x3cm. And about left hydrosalphinx, if she is married it might interfere with her pregnancies, so intervention is needed.

It's a hemorrhagic cyst of of size 4 cms, she can observe for its resolution with follow up scans for 3-6 months.
