Whenever I hold something tightly the skin is damaged it is red in colour and it's like clot.... And itching

You may be suffering from pressure urticaria. Due to pressure, chemical called histamine is released in the skin which causes these rashes which we call wheals. It can be treated with anithistamines. Please contact me in private chat for further details.
