Foreskin issue with Rashes & fungal infc

White ring appearing around foreskin tip ( looks like fungal infection), also making it painful to pull the skin back. Affected skin is not soft like before. don't know which doctor to meet general physician or sexologist? And suggestion for treatment? Should i go for circumcision ?
