Fistula reoccurring after surgery

I had gone through a surgery a year back for fistula in the ano and it got healed in a few months. However, I have now got another fistula beside the previous one and this one is hurting worse. What is the cause of reoccurrence of this fistula and which medical department should be consulted for this?
