Fever and Abdominal tuberculosis

My sister was diagnosed for abdominal tuberculosis after testing sample taken during exploratory laparotomy on 24th August Surgery was done due to bowel loops and enlarged mesenteric lymph nodes on CT. Then she was on NPO for 10 days and was given TPN. After 4 days of surgery she was started on ATT. After 7 days of starting ATT, she started to take food by mouth. After that she started developing fever everyday about 102 F. After giving paracetamol, fever reduced and after few hours increased. Before that, she never had fever except for once 2 days before surgery. From 27th September, she's getting fever everyday around 103-104 F at around 11am after eating tb medicines and goes away by 6pm without taking fever medicine. But for the last 3 days, she's getting fever at about 3pm instead of 11am  with same temperature and goes away by 8pm without fever medicine. Doctor is saying we'll see. Please tell why its happening and if its normal.

Abdominal Tuberculosis or Koch's abdomen responds to Anti TB treatment slowly, it means fever responds in about a month's time. However there could be secondary infection.For whiich other tests like blood cultures, Urine culture CBC etc are required.
Next Steps
Maintain temperature chart. High protein diet, good hydration required.
