Far vision diplopia

When I see far images, they don't tend to converge and a shadow image of the object forms above it with the image being clear. Also in sunlight, the problem becomes pronounced. My eye power is as : Left 6/6 Right 6/12. No problem in reading books a bit on laptop. Please help me!
