Extreme chapping/cracking of lips

Hi, I have chronic lips chapping/cracking/bleeding on the edges and corners of lips and many times the whole lips get dry/chapped. I have never taken this seriously but lately the frequency has increased so much that in a year hardly there is a period when my lips are not chapped. Some times i have small itchy bumps on upper/lower edge of the lips.
