Hello I have this issue since many year In my thumb nail usually a lot of dead skin kind of things gets developed under the tip of nail and also nails comes out from its nail bed. This happens and then after 2-3 month again get recovered little bit then again it happens & this process is keep going on since many years In attached image This is slightly recovered but when it  happens half of My nail came out from its nail bed.
