If he is currently asymptomatic he just needs to be on medicines and diet Control.
If he becomes breathless, or has decreased urine output, generalized swelling of body, high potassium level, acidosis, drowsiness he will need dialysis, regardless of the exact
creatinine level.
There are few other indications for dialysis, like decreased appetite, generalized itching of body,, nausea, vomiting.
Currently he looks like he needs to be on regular followup with a nephrologist. If and when creatinine goes above 7 mg/dl, it will be better to create an AV fistula.
