Simple cyst is of no worry . It might be a follicular cyst . We need not do anything for that . But bowel wall thickening and free fliud in Rif could become serous if untreated. It could be pelvic inflammatory disease, appendicitis, tuberculosis or inflammatory bowel syndrome. She needs thorough history and examination, followed by cect whole abdomen +/- colonoscopy
