Clexane injection after surgery

My mother has had an esophagogastrostomy 20 days back. She used to take warfarin for 11 years because of her thrombosis ( DVT) surgery. Although because of her esophagus surgery, doctor suggested to take Clexane injections until she starts eating from her mouth. Now she's taking soft food comfortably. Should we stop Clexane and move back to warfarin? Her body is filled with black spots because of injections.
