Can't focus objects seen double

My power is -6.00both the eyes.and age is 34, I am experiencing that I am not able to focuse object from both eyes ,obiect seen double after aprox 5 feet. and if I close my one eye I see object very clealy from past 15-20 days.,can anybody suggest me to get rid of this. Is it something that I need to worry about? What precautions should I take
