Breast Cancer surgery needs to be done

She is a cancer patient, her 5 chemo are done we hv been struggling for her surgery past 1.5months due to covid everything is closed, all the tests are done in one hospital but due to ECG changes it requires cardiologist clearance, she has diabetesthyroid BP, taking Ecosprin as well (in dec 2019, she had chest pain due to blood thickness but it was not heart attack since then taking it) Oncologist suggested to have dobutamine stress echo but m not sure if that suits her, because she got vaccinated yesterday and she will be taking Inj Filgrastim 300mg for TLC on Mon tues. She is weak and her body reacted to aprepitant as well so m not sure if this suits her Ecg have been examined 4 times before as well and it was never an issue, however, her 2D echo is normal, just wanted to know if this test is required or not as this test is very costly and PAC dr just wanted a clearance from cardiologist and here in delhi OPD are closed. Please advice what to do in this case is it really necessary?

Your ECG shows RBBB. Ideally any elderly patient above 45, diabetic or hypertensive should go for stress echo or dobutamine stress echo depending upon patients physical capacity or ability to work. It's to rule out any cardiac abnormality which a person's hear can face during stressful condition such as surgery. If the patients blood pressure, haemoglobin,and kidney function test are normal it's ok to conduct the test.
