SDH noted in parieto-temporal lobe (7mm thick) and extra axail bleed along convexity (5 mm thick). Right side midline shift for 3mm. Fracture of (R) pariental bone extended into mastoid & squamous part of (R) temporal bone.Multiple hemorrhagic confession n perilesional edema in (L) temporo-pariental lobe and basifrontal region

If the midline shift is less than 5 mm, no surgery is required usually. But the decision is always taken in context of clinical findings, repeat scan findings. Admission to ICU is mandatory. Medications like anti-epileptics, anti-edema measures and other supportive care is required.
