I have azoospermia. Further evaluation suggests that left sided grade 4 and rigjt side grade 2 varicocele found. Kindly advise.

varicocle surgery may provide some benefit...
Some sperms MAY return to ur semen
However u should keep in mind that ur sperm count is unlikely to become completely normal or even near normal
If sperms return to ur semen ( in next 6 months) then they can be used for test tube baby ( IVF/ICSI)
If they don't return even after surgery -- u shall require mTESE (harvesting sperms from ur testes) and ICSI
If this fails-- donor semen can be used for IUI ( if ur wife's reproductive system is normal)
If ur wife's age is also 35 or close and /or if IUI is not likely to succeed, doctor would recommend IVF with donor sperms
Other cheaper options compared to IVF/ testtube baby----
1. Donor IUI
2. Adoption
3. IVF using extra embryos available with IVF clinic
