My uncle had urine test where ACR ratio is 5000 . Legs are swollen . He is diabetic and high BP patient having medications. What can be concluded from this result ?

This case is suggestive of diabetic nephropathy. More meticulous control of Diabetes, Hypertension, lipid status is required.
