Achalasia patients often have cough because of aspiration ( more so while lying down).
Post dilatation it may be because of :
Incomplete muscle tear leading to stasis still being there
Overdo - leading to reflux
Remember- dilatation is NOT a procedure of choice especially in younger age groups
Next Steps
1. Consult medical / surgical gastroenterologist and get re-evaluated including endoscopy, manometry.
You may need definitive intervention like endoscopic POEM or laparoscopic Heller’s myotomy
2. Sometimes it is because of reflux following dilatation/ surgery/ POEM - in that case syp like digeraft may be of help. If it still persists then laparoscopic partial fundoplication might be of help
