White patches on skin- Fungal Infection?

I have got white patches on my neck and sholder area. This has recurred to me after 5 long years. Don't understand the reason though to occur after 5 long years. Has been continuing the cream and dusting powder for the same from last one month, but it doesn't seems to be reducing. A snap has been attached. Clotrimazole Powder and Fourderm is the cream which I had used earlier I am continuing with the same. But see no improvements.
