Pharyngitis or sore throat involves the following:
Symptoms â enlarged tonsils, tonsillar exudates, cervical adenopathy, fever, rash.
Physical Exam â oral cavity, tonsils, may show erythema, exudates
Diagnosis â
⢠CBC (inc WBC)
⢠Throat Cx (positive for strep, influenza)
⢠Rapid strep, mono spot for EBV
Treatment â
⢠Penicillin V 500 mg PO BID x 10 days (Penicillin G 800,000 units PO BID x 10 days). For children < 27 kg, give 250mg), OR
⢠IM Penicillin G 1.2 million units x 1. For children < 27kg, give IM Penicillin G 600,000 units x 1.
If allergic: amoxicillin. Secondary options include: azithromycin, calrithromycin, erythromycin, cephalexin
⢠Acetaminophen (fever, pain): 325-1000 mg PO q4-6 hours PRN, maximum 4000 mg/day
⢠Ibuprofen : 200-400 mg PO q6-8 hours PRN, maximum 2400 mg/day
Alternate Acetaminophen and Ibuprofen Q3 hours
⢠Chloraseptic spray
