Spine problem (Abutting of nerve root)

I have chronic pain in the left leg. I got an MRI done of lumbar-sacral spine on my doctor's recommendation. The MRI report says that Postero-central inferior migrated extruded disc is detected at L4-5, abutting the left traversing nerve root. Also, transitional lumbar vertebra is noted with sacralization of L5. Should I go for a surgery or will physiotherapy be enough?

Lubrication or nourishment is not proper to discs ..so the sacralisation... good to find the cause in lifestyle and get a correction... and as per age it corrects by internal and some external treatment measures of Ayurveda.
