Hi dear, as per your symptoms and the endoscopy findings this looks like a chronic duodenal ulcer which led to narrowing of the 1 st/2nd part of duodenum.
Before surgery a CT SCAN OF ABDOMEN WITH ORAL AND I.V CONTRAST is must to define the lesion/ its extent and outside lumen picture.
Tuberculosis should be ruled out.
If after all diagnosis excluded then a Laparoscopic Truncal vagotomy and a Gastro-jejunostomy is the choice of surgery, where you will find it as a scarless surgery ,we work with a 5 mm size of 3- 4 holes.
