Small cracks on penis outer skin. S

Once in few months I have been experiencing small cracks on my penis skin. This makes difficult for erection and it pains if I try to indulge in sex. Need to understand why this happens? How can I avoid this? I have been applying Nizral and Nadoxin cream but no progress. Pl let me know how this can be treated
