Since you had severe LV dysfunction, you were correctly advised fluid restriction. Now that you mentioned your LVEF has improved, it's only mild LV dysfunction ...you can relax the fluid restriction and see if your constipation improves.
All the medicines you are on are absolutely correct and required in DCM, of course titration is based on your follow up clinical check ups and blood work. Plz continue the medications, even if the LV function has improved, you are required to continue them
Perfusion defects on PET scan can be either infarction (heart attack related due to blockages ) or inflammation ( myocarditis post any viral or other infection or sometimes no trigger is specifically there) or fibrosis (scarring of the heart muscle can be post myocarditis or some other systemic diseases)
Since your Coronary angiogram is normal the possibility of infarction is not there.
It's either inflammation or fibrosis. Usually fibrosis doesn't improve, but your heart function has improved. Had you been given steroids anytime for this? Since it's improving we can be hopeful that it will improve further provided you continue all medications. Unfortunately no time line can be given as recovery period varies from weeks to months to years.
