Patient is a 28-year-old female with severe headache, neck stiffness, spine and low back pain, recurrent dizziness, sensitivity to light and noise, nausea, pain while swallowing, difficulty moving the neck, disturbed sleep, repetitive thoughts, emotional distress, crying spells, difficulty understanding conversations, and anxiety/stress. Consulted Neurologist on 05 Jun 2026 and diagnosed with migraine. Treated with Provanol SR 40, Atreep 10, Topaz 25 and Spinfree with only partial relief. Later consulted MD Medicine on 10 Jun 2026.
