Post ZSR surgery swelling

I went through ZSR circumcision and it's been 21 days. Yet the Silicon ring has not fallen off and pins are dislocated from the ring and causing hurt. Still I am applying Metrogyl Gel 2% as prescribed by doctor. Please help me do I need to consult doctor or suggest me any solution
