An ENT doctor will help you medically as well as with lifestyle modifications, to get the GERD under control as well as work on the allergies. The size of the polyp could be reduced giving short course steroidal anti-inflammatory drugs after which re-evaluation will be done.
Eventually a surgery would be needed after getting fitness from the pulmonologist.
