A 7 mm Subdural hematoma with 5 mm midline shift is significant. It definitely needs surgery. Although there is some risk in Surgery, if left alone this will slowly expand and can cause weakness of hands and legs and even threat to life due raised pressure. Better to go for a minimally invasive surgery like 2 burr holes and removal of the clot.
