When a foreskin stays retracted behind the head (glans) of the penis for a long time and cannot easily be brought forward, it is typically a sign of paraphimosis. If a surgeon previously mentioned fibrosis, it means that chronic inflammation or micro-tears have healed as tough, inelastic scar tissue. This scar tissue creates a tight, restricting ring that essentially gets "anchored" behind the ridge of the glans.
