It is most importantly essential to rule out an allergic reaction to a new  medication prescribed for PCOS.  A severe or rapidly progressive Folliculitis associated with PCOS,  may be due to inherent peripheral resistance associated with the disease. Her sugars need to be checked to rule out a new onset Diabetes Mellitus which would predispose her to severe skin infections. In addition a skin sawbuck from the lesion would definitely confirm the causative organism and it's antibiotic sensitivity. Acanthosis Nigricans is another non infective skin issue which is related PCOS,  but resolves with blood sugar control. Common communicable diseases like scabies also needs to be ruled out.
