I’m reaching out to get a second opinion regarding a varicocele issue that is causing me ongoing pain and discomfort. An ultrasound Doppler report shows: Grade III/IV varicocele on the left side Dilated pampiniform veins >3 mm Reflux present both at rest and during Valsalva maneuver I’ve also been experiencing: Visible swelling of scrotal veins Dull pain in the scrotum and lower abdomen Pain worsens while standing for long or walking A local urologist has recommended microsurgical varicocelectomy, but I would like to know your expert opinion before proceeding. Could you kindly help me with the following: Do you think surgery is necessary in this case? Is microsurgical varicocelectomy the best approach for me? Are there any non-surgical options I should consider first? What are the risks and long-term results if I delay or avoid surgery? Can I consult you in person, or do you offer video consultation for second opinion?
