Bone flap removal is done to reduce the pressure in the brain and to increase the chances of survival. This surgery makes a window in the skull and hence the pressure is released which otherwise may compress the brain and may be life threatening. Replacing the bone flap after few weeks restores the skull cover.
This surgery usually does not restore the function if the brain lost due to stroke and hence there is no direct recovery of the paralysis.
