Can the area around the white spot be a little red? Can it be tonsil stone in pharyngeal wall? When I shout or have dry throat then the area gets irritated more

If white debris of keratosis get calcified then it's known as tonsil stones. Usually cause for keratosis is reflux laryngopharyngitis which is type of acidity related disorder. This can be the reason for dry throat. You may need to take anti reflux medications for few weeks
