Itching and red rashes on head of penis

Since 2 months I guess I have a problem of this itching and red rashes on head of the penis and under folds of penis skin. Sometimes I see white substance also. I used fourderm ointment and it goes off but again after few days it's show same itching and redness. Please advise me any permanent solution. Thank you
