Issue with Internal Sphintronmy

My brother underwent surgery for a fissure problem. Lateral internal sphincterotomy was performed five months ago. Two months post-surgery, he started experiencing white discharge from the 3 o'clock position where the surgery was performed, accompanied by pain. I can also see a raw area which is becoming inflamed. Despite consulting multiple doctors  symptoms diagnosed fissures, the issue persists despite medication. What could be the reason for the discharge, and is it something serious to be concerned about? What treatment should we seek to resolve this problem? He is becoming very depressed.
