Hep C cured, Hb and Na dropping

My mother underwent Hep C treatment but soon after she had to be readmitted with low Hb, where she was given 3 units of blood to raise the Hb from 7.6 to 10.6. Once she came back home in a week's time the Hb came down to 9.3 and Na to 132. She hardly has apetite, and no strength in her body. Very drowsy and sleeping almost all day. Please advise.
