I got an injury in head 4 years back for which I had to get stitches in left part of my head. Though its been a long time but still I can feel the pain there. Also, since then I have lost quite good amount of hair from my left portion of head and also my left eye has got power of +1. On concentrating or sometimes in stress, I feel heaviness and a bit of pain in left part of my head. Is this a symptom of any disease or do I need a CT scan?

Yes you need to do a CT Scan to find out if you had any internal injury at that time and then see a neurologist for proper evaluation and treatment.
