Flesh chunks from Vagina

This morning when she was cleaning her self down there , she discovered small pinkish pieces of flesh coming out of her vagina. Although it stopped coming after a while but this was something really new and had never happened in the past. We are sexually very active so could this be because of that or is it something else. Should we be worried or is it normal. Kindly advice.
