Hello, it seems like your are having refractive errors of eye (probably hypermetropia). Which makes you unable to focus your eye for nearer objects, and trying to focus cause you experience, headache which in turn makes to lose concentration. All you have to do is visit nearer ophthalmic centre or eye hospital, get tested for refractive errors(which is a 5min OPD procedure).
