Four years back to my right ear i had a tympanoplasty and everything was good, since from last two years during winter season i"m getting middle ear infection and my doctor use to give antibiotics to drain my ear.. but last month i taken my CT scan and found there is an infection in my mastoid air cell.. i consulted 4-5 surgeons and one surgeons and found these three options. 1. go for septoplasty OR 2. go for both nose & ear OR 3. go for ear only

You seem to have had failed tympanoplasty which is common with ear surgeries as many times post operative infections can lead to failures. You may need to undergo tympanoplasty with Mastoidectomy procedure. Usually nose surgeries are not usually necessary
Next Steps
Consult nearby experienced ear surgeon for more information about the surgery and it's outcomes
