Constant pain in premolar tooth

I am suffering from tooth pain from last two month. My right side of the first premolar and Second premolar tooth pains continuously in the night when i sleep. it also pains when i eat some sweets. Both the tooth are not decaying but i think there is some small holes are taking place in both tooth. plz suggest me what to do
