Microscopic Description
Sections demonstrate skin with hyperkeratotic orthokeratosis. The
epidermis shows subtle areas of wedge-shaped hypergranulosis.
Focally, necrotic keratinocytes or "colloid" bodies are noted, and
a dense lichenoid infiltrate of lymphocytes with scattered
melanophages partially obscures the dermal-epidermal junction. The
histologic features represent a lichenoid dermatitis. The
differential diagnosis includes lichen planus and a lichenoid drug
eruption.
