Anal fissure with severe swelling

This is the second time I'm having anal fissure, first time was last year. This time after every bowel movement I can feeling swelling which protrudes out at first and then after some time it subsides/goes inside. I have been using proctosedyl cream which gives temporary relief but now the pain and swelling has increased alot. Please help
