Seems to be a case of Gastro esophageal reflux disease. Get a Barium swallow and a GER scan. The feeds should be thickened and small  volume feeds should be given at frequent intervals. Baby should be made to lie prone or an upright carried position after feeds should be maintained  to prevent reflux. Syp Rantac / lanzol junior should be given regularly. Severe cases may require surgery

Your baby has undergone too much in Past 6months owing to his low birth weight and surgery..  We need to look at your baby's growth chart to monitor ur baby's growth. Baby's usually have immature tone of their muscle which holds the food in stomach. This tone usually starts getting better after 6months..
Next Steps
Consult a Pediatric Gastroenterologist for work up if weight gain is poor.
Health Tips
1. Raise the head end of baby's bed by 30 degree. 2. Give good 15-20min of burping post feeds. 3. Increase the calorie content of feed and make it more thicker. 4. Give Small frequent feeds rather than large feeds.
