Got a root canal done in my back molar about 5 years back and now it is hurting. There's no sensitivity but there is pain. Hurts when I clench or bite down on it. Otherwise no pain. The gums surrounding it are slightly swollen. What could the issue possibly be? Do I have to redo the root canal treatment again? Is it a infection or a fracture?

Hello, pain could be because of various reasons like high filling or remnants of the tissues, you need to visit the dentist and get it checked. if the filling is high the dentist will reduce it and if there are remnants left behind, the dentist will do the treatment accordingly.
Hello! All the symptoms you have mentioned indicate a periodontal (gum) problem. The tooth has been asymptomatic for 5 years so there are less chances of a tooth infection. Swollen gums indicate gum infection along with the kind of pain you have explained. You mostly have developed periodontitis, localised if it's only that tooth with swollen gums around it or generalized if most of your teeth have swollen gums, not necessary pain in other teeth though!
Have you got a cap placed on the root canal treated tooth? If not then there could be a possibility of a fractured tooth.
Next Steps
You should visit a dentist & get a scaling (cleaning) done along with an x-ray (IOPA) of the particular tooth to rule out any infection / fracture or other causes for your discomfort.
Health Tips
Use a mouthwash as an adjunct to brushing. Get a scaling done at least once a year, if possible every 6-8 months. Please get caps for all your root canal treated teeth.
As per your query, their is high possibility of high filling because of which there is problem on biting. you are advised to visit a dentist and get complete diagnosis after xrays to get the root cause.

Hello Mam,
As A Root canal specialist I would advise you to get an x-ray done to re-evaluate the Root canal treated tooth and the periapical area ASAP.Looks like an abscess is developing in the area.Regards,Dr Sumanmakam,Dental arts, jayanagar,Bangalore.
Hello,
As per your symptoms, there may be high filling because of which there is problem on biting. But as it was having no pain earlier for 5 years. and the pain has just started, it will be better if u get it checked once. and lets take an xray and see whats the problem and how was the rct done. that will give us an idea whether there is an infection problem of teeth or a gum problem. till then plz avoid eating from that side. and do salt water rinse.
Hello there.
I can understand your concern.
You can take a tab. Zerodol-p twice a day after food for now.
Yes it could be a infection.
But without seeing we cannot say anything for sure.
Consult a dentist in person for further evaluation and management.
Best regards.
