Hi!
Fungal infections of the nail need to be diagnosed by a simple investigation known as KOH or a culture. It usually needs oral antifungals for treatment as topical creams and lacquers can not penetrate the thick nail plate easily.
Many nail diseases can present in the way you have described so it is best to meet a dermatologist so that proper medication may be started.
