Your issue is quite common among uncircumcised males. Whether or not it needs any surgical intervention, depends on how painful it is during sexual activity. Penile foreskin is very flexible, and can be stretched gradually, either by progressive manual pull, or by frequent sexual intercourse. Does your pain subside with additional lubrication? Are you able to have normal intercourse? If the answer to these to questions is yes, then you don't need to be surgically operated on. If, on the other hand, the answer is no, I recommend you consult a Urologist for further assessment of your concerns. Good luck.
