Your symptoms of lethargy, erratic sleep pattern and cognitive symptoms like lack of attention, concentration and forgetfulness can happen because of various medical and psychological conditions. You will need to see in person to get detailed history, physical examination including some blood tests like Sugars, B12, Vit D etc. Once physical causes are ruled out, psychological conditions like depression and anxiety are considered which are treatable with effective psychological treatment with or without medicine.
